Add fixes and cleanups

Got rid of \startappendices and \startpages. These are not really needed and add confusion.

The document parameter emptyfirstpages hides headers/footers of the 1st pages of the chapters.

Cleaned up options processing, the .tex template and few bug fixes. Added more test cases, both for pdflatex and xelatex, en/fi translations and hyperref.
